How do I replace my badge?

Please complete the YPS Identification Badge Request Form to request a replacement ID badge. We will need to reissue another badge & ID # and deactivate your old badge. Pictures are saved in the ID system. You don’t have to have another picture taken. For security reasons, you will be required to pick-up your badge from the Office of Technology & Information Services (OTIS) in Room 1020 of the Administration Building. We will notify you via our support system when your badge is ready for pick-up. The cost to replace a badge is $5.00.
